* A group health plan that is a multi-employer plan can specify
by its terms that the employer or plan administrator is to have
a longer period of time to give notice.

Continuation for a maximum of 18 months
(termination, reduction in hours) or 36 months
(death, Medicare entitlement).
18 month continuation can be extended to 29 months
if disability occurs at any time during the first 60 days of COBRA coverage.
